We'll take this one step at a time 

We're committed to making this process as simple and supportive as possible during a difficult time. Starting a claim takes about 15 minutes and it's a bit easier if you have your loved ones:

  • The policyholder's full name and date of birth
  • Equitable policy number and or Social Security number
  • Death certificate or obituary link
  • Funeral home contact information and any documents provided

What to expect during the claims process

We understand this may be a difficult time. The information below will help you understand the claims process, including key steps and timing along the way.

Notify Equitable 

The first step is to let us know about the death and start the claims process

Before you start, please have available:
  • Full name of the deceased
  • Date of death
  • Social Security number for the deceased and beneficiary(s) (Required)
  • Beneficiary information
  • Policy/contract number (If available)
  • Policy/contact information for the person notifying Equitable of a claim

What happens next?

After you submit your online notification:

  • You’ll receive an email confirming that we received your information immediately after submission.
  • Our team will review the details provided in your notification. Once reviewed and authorized:
    • You will receive a second notification with:
      • A unique “Claim ID”
      • Instructions for registering and tracking your claim online.
    • In addition, we will send the appropriate claim forms (claim kit) to each beneficiary, named in the policy/contract, within 5 business days following the initial notification.

We'll send you the forms you need in a “claim kit"

We'll mail or electronically deliver a personalized claim kit to each named beneficiary. In most cases, claim kits are sent within 5 business days, following the initial notification.

Your claim kit includes:
  • Required forms needed to complete your claim
  • Form completion instructions
  • Return mailing information
Track your claim online

Once you receive your Claim ID (by email), you can register for an account to:

  • Check claim status and updates
  • Track payments

If you do not receive your claim kit call:

Submit your completed claim forms

Follow the instructions included in your claim kit and return all required forms and documentation. Please ensure forms are:

  • Complete
  • Signed where required
  • Submitted along with any requested supporting documents
  • Return instructions and mailing addresses will be included in your claim kit.

Review of your claim 

Once all required forms and documentation are received and in good order, most claims are processed within 5 business days. 

We’ll contact you if we need any additional information including:

  • Missing information
  • Incomplete documentation
  • Additional review required

Note: some claims may require further investigation and can take additional time. 

Receive your payment
  • Claim benefits are paid according to the beneficiary's payment instructions.
  • Following the review, you should receive your payment within 5 business days. 
  • For payments made by check, please allow 3–5 business days for mailing and delivery.

Track your claim status

Log in to your account using your Claim ID:

  • Monitor progress
  • View updates
  • Check payment status
  • Access claim information
Your privacy matters

To protect confidential information, we communicate only with:

  • The beneficiary
  • The beneficiary's authorized attorney
  • The servicing financial professional
